CONGREGATION ALBERT - MEMBER INFORMATION UPDATE

This form allows members of Congregation Albert to provide updated information for use in the Congregation Albert database and our yearly directory. If you are a member, and IF YOUR INFORMATION HAS CHANGED, please take the time to complete the information requested below and submit it to us electronically. Adults with different last names will be listed under both names. The directory also includes the names of children currently living in a member's household. If there are more than two adults and four children living in your household, please print this form multiple times and submit it by mail instead of using the electronic version.

Please submit the completed form to us as soon as possible after your information changes. Your prompt response is very important to insure timely publication of our new directory and the accuracy of the Temple database.
