Member Information Update - April 2000

CONGREGATION ALBERT - MEMBER INFORMATION UPDATE

The Temple is in the process of updating its database and preparing a new directory for distribution in late summer. If you are a member, please take the time to complete the information requested below and submit it to us electronically. Adults with different last names will be listed under both names. The directory also includes the names of children currently liing in member's households. If there are more than two adults and four children living in your household, please print this form multiple times and submit it by mail.

Please submit the completed form to us no later than May 20, 2000. Your prompt response is very important to insure timely publication of our new directory and the accuracy of the Temple database.
